What Are Wrinkles?
Two Very Different Problems
Wrinkles form through two separate mechanisms, and telling them apart is the single most useful thing you can do before choosing a treatment. Dynamic wrinkles are created by muscle movement — every time you frown, smile or raise your brows, the skin folds. Early on these lines vanish the moment your face relaxes. Static wrinkles are visible even at complete rest, caused by the gradual loss of collagen, elastin and facial volume, accelerated by sun damage.
Over the years, dynamic lines gradually etch themselves into static ones — a frown line that once disappeared starts to stay. This is why treatment differs so much: relaxing the muscle helps a dynamic line, but a static line that has already formed usually needs volume restored or collagen rebuilt. A quick test at home: look in the mirror with a completely relaxed face. Whatever you can still see is static.

Dynamic (Expression) Lines
Frown lines between the brows, forehead lines and crow's feet around the eyes. Caused by muscle movement and visible when you make an expression. The classic Botox territory.
Static Lines
Still visible when your face is completely relaxed, caused by collagen loss and sun damage. Need volume, collagen stimulation or resurfacing rather than muscle relaxation alone.
Fine, Crepey Lines
Delicate surface lines from dehydration, thinning skin and sun exposure — often around the eyes and cheeks. Respond well to skin-quality treatments and a good retinoid routine.
Folds & Sagging
Deeper creases like nasolabial folds and marionette lines, driven by descent and loss of facial volume rather than the skin itself. Need lifting or volume restoration.

Relax your face completely and look in the mirror. Lines you can still see are static. Now frown or smile — lines that appear only then are dynamic. Most people find they have both, which is exactly why a single treatment rarely addresses everything and a combined plan gives the most natural result.

Dynamic wrinkles appear when you make an expression — frowning, smiling, raising your eyebrows — and are caused by repeated muscle movement. Static wrinkles are visible even when your face is completely at rest, and are caused by collagen loss, volume loss and sun damage. Over time, dynamic lines gradually become static ones. This distinction matters because Botox relaxes the muscle behind dynamic lines, while static lines usually need fillers, collagen-stimulating treatments or resurfacing instead.
It depends on the type of line. Dynamic lines on the forehead, between the brows and around the eyes respond best to Botox. Static lines and folds respond to dermal fillers that restore volume. Sagging and loss of firmness respond to HIFU or a thread lift, while overall skin quality and fine crepey lines improve with RF microneedling, laser resurfacing and a good retinoid routine. Most people get the best result from a combination, which Dr. Sania Khan plans at consultation.
There is no fixed age — it depends on your skin, genetics and how much sun exposure you have had. Prevention can start at any age with daily sunscreen, which is by far the most effective anti-ageing step. Many people begin considering treatments in their late twenties to thirties when dynamic lines start to linger after the expression stops. There is no benefit to starting before you actually have a concern.
No treatment removes wrinkles permanently, because skin continues to age. Botox typically lasts three to four months, fillers many months to over a year depending on the product and area, and HIFU or thread lift results last around a year to eighteen months. Results are excellent but maintained with repeat treatment. Daily sun protection is what genuinely slows new wrinkles from forming.
Partly. Botox relaxes the muscle that creates the line, so a deep static line will usually soften but may not disappear completely on its own. Deep etched lines often need Botox combined with a filler or a collagen-stimulating treatment to fill and rebuild the crease. This is why an assessment matters — being told honestly what one treatment can and cannot achieve avoids disappointment.
Sun exposure is the single biggest cause of premature wrinkles, accounting for a large share of visible facial ageing. Smoking, poor sleep, dehydration, repeated strong facial expressions and genetics also play a role. In Pakistan's strong sun, daily broad-spectrum sunscreen makes a very significant difference to how early lines appear.
Some ingredients genuinely help — retinoids have the strongest evidence for improving fine lines over months of consistent use, and sunscreen prevents new ones. However, over-the-counter creams contain lower concentrations than prescription products and cannot replicate what injectables or energy-based treatments do for deeper lines and volume loss. Creams are a valuable foundation, not a substitute.
Botulinum toxin has been used cosmetically for decades and has a well-established safety profile when administered correctly by a qualified medical professional in appropriate doses. Risks such as drooping or an unnatural look usually come from poor injection technique or unqualified injectors. Skin with more melanin generally has some natural protection against photoageing, so deep wrinkling often develops a little later than in very fair skin. However, pigmentation changes and uneven tone tend to show earlier and are frequently the more prominent ageing concern. Treatments also need adjusting for darker skin tones, particularly resurfacing lasers, where conservative settings reduce the risk of pigmentation.
